In consideration of the Hometown Christmas Parade and Goosebump Marketing & Events (the "Event Producers’) agreeing to interview me and/or photograph me and/or my property, I hereby grant my consent to the Event Producers in perpetuity to use, reuse, sell, edit, excerpt, publish, republish, individually or in conjunction with other material of any kind, my name and my likeness, my photograph and any other reproductions of me and/or my property, including my features, with or without my name, together with any alterations or additions thereto (collectively, the “Material") for any publishing, editorial, promotion, advertising, trade, business or other purpose whatsoever, except for the endorsement of any product. This Consent applies to the use, reuse, sale, publication or republication of the Material in all communication mediums. The Event Producers may exercise its rights granted herein in any way it sees fit without limitation, reservation or payment of any fee. A.R.S§ 1-602.A.9: Notice is hereby given that pursuant to A.R.S. § 1-602.A.9, subject to certain specified statutory exceptions, parents have a right to consent before the State or any of its political subdivisions to make a video or audio recording of a minor child. Recreational activities and programs may be video recorded and, as a result, children present may be subject to such taping. Signature by a parent or guardian gives the Event Producers written consent to videotaping and recording, and the Event Producers will assume that the rights afforded parents pursuant to A.R.S. § 1-602.A.9 have been waived.

ATTENDEE WAIVER: By submitting this form, I acknowledge that I have received a copy of the parade rules and regulations for the Hometown Christmas Parade. In consideration for permission and authority to participate in and be a part of the Parade, do hereby release, waive and discharge in connection my / our participation in the parade. I knowingly enter into the agreement and understand the significance and consequences. I understand that as a participant in the parade, I can be injured by a fall, actions or inactions of other parade participants or bystanders, imperfections in the street, sidewalks or curbs, hazards from motorized vehicles, and all other hazards. I enter into this release and waiver on behalf of myself and my heirs, descendants, ancestors, dependents, executors, administrators, and assigns.

INSURANCE REQUIREMENT: The Entrant, by his authorized signature below, hereby agrees to indemnify, hold harmless, and defend the Hometown Christmas Parade & Goosebump Marketing & Events, and appointed officials, employees, agents and successors in interest from all claims, damages, losses and expenses including attorneys’ fees, arising out of or resulting, directly or indirectly, from the Entrant’s participation in the Hometown Christmas Parade, provided that such claim, damage, loss or expenses attributable to personal injury, bodily injury, sickness, death, or to injury or destruction of property, including the loss of use resulting there from, this Hold Harmless and Indemnification Clause shall in no way be limited by any financial responsibility or insurance requirements and shall survive the termination of this Parade Entry Form.

IT IS MY SPECIFIC INTENT TO RELEASE GOOSEBUMP MARKETING & EVENTS; HOMETOWN CHRISTMAS PARADE AND ITS VOLUNTEERS, ITS DIRECTORS AND OFFICERS FROM ANY LIABILITY WHATSOEVER IN CONNECTION WITH ANYONE AND/OR MYSELF IN MY GROUP IN THE PARADE.
